- The plan must be current in order to receive included or discounted services.
- The plan runs for 12 months. You will receive an email reminder 45
days prior to the renewal date. Auto renewal is managed through your member portal. - Policy applies for 1 year term. Cancellation of policy in less than 1
year will result in any discounts applied being removed. Patient will
be responsible for all discounted portions if policy is cancelled prior to 1 year term.
